This MISD Form 2 Handbook guides procedures to initiate services and supports for students with special needs in Macomb County.  The content has been modified to meet current practice and begins with the 2007/2008 school year.

 

Contained within the guide are the following: Assessment Systematic method of obtaining information from tests or other sources; procedures used to determine child’s eligibility, identify the child’s strengths and needs, and services child needs to meet these needs. See also evaluations. and Clinics, Educational Consultation, Support Services, and ISD or LEA-Based Classroom Programs.  A final section addresses forms and memos.

 

These procedures are for LEA Local Educational Agency, i.e., local school district. Directors, Supervisors and Center Program Administrators or their designee, to seek necessary assistance for individual students with special needs.  Any requested service or support requires a Form 2 filled in its entirety, along with the parent’s and director’s signatures, and all supporting documents/forms to be processed.

 

Macomb County continues to provide excellent services through the Form 2 process with the ultimate goal of promoting successful instruction for students with special needs.
