The Student Follow-Up service consists of procedures aimed at
surveying all persons who are no longer receiving any
special education programs or services, i.e., special education leavers.
It is intended that results from this follow-up survey will be used to
determine the effectiveness of special education programs and services,
and to make modifications and improvements based on identified needs.
Areas of investigation:
While looking at the school-community adjustment of persons with
special needs, ten specific
areas of concern are surveyed. These are important indicators of adjustment.
They are included here to aid in the understanding of the survey as well
as to guide future refinements and additions. The Follow-Up Survey is
intended to investigate the following major areas:
| o Independence |
|
o Health practices |
| o Use of leisure time |
|
o Self-esteem |
| o Mobility practices |
|
o Rehabilitation |
| o Personal/social adjustment |
|
o School/Job success |
