The Student Follow-Up service consists of procedures aimed at surveying all persons who are no longer receiving any special education programs or services, i.e., special education leavers.  It is intended that results from this follow-up survey will be used to determine the effectiveness of special education programs and services, and to make modifications and improvements based on identified needs.


Areas of investigation:
While looking at the school-community adjustment of persons with special needs, ten specific areas of concern are surveyed.  These are important indicators of adjustment. They are included here to aid in the understanding of the survey as well as to guide future refinements and additions. The Follow-Up Survey is intended to investigate the following major areas:

o Independence o Health practices
o Use of leisure time o Self-esteem
o Mobility practices o Rehabilitation
o Personal/social adjustment  o School/Job success
o Habilitation  o Program success


Who is to be surveyed?
ALL students who have been terminated from ALL special education services of the MISD or its constituent school districts are surveyed.

How is the survey conducted?
A personal interview is conducted with the use of MISD Special Education Follow-Up Survey Form by telephone. If the surveyor is unable to make a phone contact, then a mailing is attempted to get a response.
   o Out-of-school leavers are surveyed by telephone; hearing impaired students who are surveyed by mail.
   o In-school leavers are surveyed by telephone to parents.

When is the survey interview to be conducted?
Within one year of termination of a special education program, the director of special education shall assigns responsibility for each follow-up interview and specifies the date for its completion. Students should be surveys no earlier than the first anniversary date of their termination from special education and before the 2nd anniversary. Some exceptions exist due to special circumstances.

Summary and analysis of data
An annual statistical summary of data is provided for each individual district as well as a compellation report of all MISD students and those of its constituent districts. These reports are forwarded to the directors/supervisors of special education of each constituent district as well as the director of special education of the MISD center programs.
