General Referral Procedure

A Form 2 requesting an evaluation, consultation, and/or service must be submitted to the MISD, Support and Related Services Department for Occupational Therapy, Physical Therapy, Vision and Orientation and Mobility Services.

• Referral materials must include:

• Copy of the most recent IEPT

• Referral Form (for Occupational and/or Physical Therapy)

• Referral must be signed by both the Parent and the Special Ed Director

• Current eye report (for Vision services)
