Purpose/Function:
Consultation for students with Physical/Other Health Impairments is provided to staff working with these students for which there is need of additional input regarding diagnosis, intervention or management.
The MISD PI/OHI Consultant assists local school district by providing:
- Observation of the referral student in multiple settings.
- Discussion with the staff of intervention strategies and procedures.
Frequency:
As determined by the consultant and the referring district.
Referral Process:
A Form 2 Referral is sent to Center Program Director.
The following documents should be enclosed with the referral:
Current Eligibility Report(s)
Current IEP
Current REED Report(s)

Current Medical Records
Psychological
Social/Developmental History
Referral Response:
Form 2A is sent to referring district to acknowledge the referral.
The consultant contacts the district staff person listed on the referral.
Outcome:
The consultant may provide:
- Observation
- Report upon request
- Staff training recommendations
- Resource information
- Accommodation suggestions
- Follow up services as needed
Exceptions:
The PI/OHI Consultant is available by telephone to assist with questions regarding accommodations and resources.
